To submit a nomination, you'll need to send us the following information:
- A) The name and address of your candidate, as well as your own name and address, for correspondence purposes.
- B) A short description of your nominee's contribution to the community, including the type of volunteer work accomplished, the names of any organizations involved, the length of service, and any other relevant biographical information.
- C) A letter from an organization or individual who supports the nomination and can speak of the unpaid volunteer work accomplished by the candidate.

There is no deadline for submissions. Nominations are accepted throughout the year.
